COVID-19 Survey
The Addiction Policy Forum invites you to provide feedback on the impact of COVID-19 to gain a better understanding of the challenges, barriers, and real-time changes they are experiencing during this public health crisis.
The goal of this online, anonymous survey is to collect feedback from individuals in recovery, active addiction and impacted family members to fill the research gap about the needs and challenges and improve our nation's response during the crisis.
What is this?
The online survey is designed to collect feedback on thoughts and experiences during the COVID-19 public health crisis to help inform gaps, barriers, and challenges. Participants will remain anonymous and no identifying information will be collected.
Who can take this? Patients, impacted families, individuals in recovery, and individuals actively using substance are encouraged to take the survey. How long will this take? This should take about 10-15 minutes to complete.
